Grants on Housing

On July 5, 2017, Partnership HealthPlan of California (PHC) released a Request For Proposals for Local Innovation Grants on Housing. The one-time grants, totaling
$25 million, are for housing and housing supports for the PHC population. In accordance with our mission, to help our members and communities be healthy, PHC has been working to address the social determinants of health – the social and environmental factors that affect the health and welfare of individuals and communities. A critical determinant of health is one's housing situation. Many of PHC's members experience homelessness and precarious or inadequate housing conditions. Through the housing initiative, PHC will work with our communities to support the health and health care needs of our members. Grant applications have been received throughout our entire 14-county region. We look forward to partnering with our communities on these upcoming innovations.

 

Status Update – 

Timing of Grant Decisions

PHC has completed preliminary review of all housing project proposals received.  Over the next two weeks, we will finalize our funding recommendations.

Funding decisions will be announced in early November.

PHC Board of Commissioners: Consumer Representatives

Do you want to have an impact on health care in your community? We are filling two consumer seats on our Board of Commissioners, one in our Northern Region and one in our Southern Region.

The individuals selected will be required​ to attend the six PHC Board meetings during the year and provide a consumer update at each Board meeting (a stipend for travel costs is included).

Additionally, participation at our Consumer Advisory Committee (CAC) meetings will be encouraged, but not required.

If you are interested in applying, please complete and submit an application that corresponds to your county of residence.

Click here for the Northern Region application if you reside in Del Norte, Humboldt, Lassen, Modoc, Siskiyou, Shasta, or Trinity County.

Click here for the Southern Region application if you reside in Lake, Marin, Mendocino, Napa, Solano, Sonoma, or Yolo County.

To apply as a consumer representative, one should:

    • Be a current PHC member or a family member of a current PHC member
    • Submit a completed application by the deadline

Applications must be POSTMARKED by the deadline date of Thursday, June 1, 2017 at 5 pm.
