An Advance Directive is a way for you to put your own wishes for end-of-life care in writing. While it is not a pleasant thing to think about, we will all sooner or later face decisions about how much or how little we want done as we approach our final days. Filling out an Advance Directive allows you to decide for yourself how much you want done. It can take a burden off your family if you have already made your own wishes clear.
