The main goal of Partnership HealthPlan of California's Care Coordination Department is to give members what they need and want through good quality health care. Members' needs and what they want should be delivered at the right time, to the right people, and in a way to guide the delivery of safe, correct, and helpful care.
Partnership's Care Coordination Department uses different levels of services to help members self-manage and get back to their best health. All levels of services include a full review of members' health and care needs and finding what benefits and resources are open to them. This is done through a plan to meet goals. These goals are met through the programs below:
