adeline

Madeline was referred to a skilled nursing facility-to-community program after a traumatic incident in which she could not return to her home. She worked with her Partnership social worker in overcoming obstacles relating to her health. Read Madeline's full story.

    Daniel

Daniel was referred to a skilled nursing facility in March of 2023 due to diabetes complications. He worked with his Partnership social worker to assess his best next steps in returning home and finding In-Home Supportive Services (IHSS). Read Daniel's full story.

    Ginger and Chelsea

In February of 2024, we partnered with one of our Family Advisory Committee (FAC) members, Ginger Moyles, and her daughter Chelsea, to bring awareness to World Rare Disease Day. Chelsea's connective tissue disorder is so rare, that there are only 50 documented cases worldwide. We're proud to share Chelsea's story and raise awareness to the 300 million individuals worldwide that have a rare disease. View the full video.

  Nunie

In early 2024, Partnership member Nunie Matta was selected to be part of the Department of Health Care Services (DHCS) Medi-Cal Member Advisory Committee. This committee is comprised of about 20 Medi-Cal recipients who offer their prespective and experience to enhance quality health care. Nunie is also a member of Partnership's Consumer Advisory Committee, which meets quarterly to review member materials and advise Partnership on how we can better serve our members. View the full video.
