Offering and Honoring Choices™
In 2012, Partnership HealthPlan of California (PHC) began the Offering and Honoring Choices™ initiative to ensure that its members and their families are knowledgeable about health care treatment options, empowered to define their treatment goals, and able to make informed choices about the interventions they choose during the last years of life.
The main Offering and Honoring Choices™ areas are:
Advance Care Planning
Palliative Care
Policy and Public Education and Engagement
Advance Care Planning is doing what we can to ensure the health care treatment we may receive is consistent with our wishes and preferences should we be unable to make our own decisions or speak for ourselves.
Palliative Care is patient and family-centered care that optimizes quality of life by anticipating, preventing, and treating suffering. Palliative care throughout the continuum of illness involves addressing physical, intellectual, emotional, social, and spiritual needs and facilitating patient autonomy, access to information, and choice.
Activities in fostering policy development and public engagement are geared toward supporting legislation related to and consumer engagement in advance care planning and palliative care.
