2025-2026 Perinatal QIP Measure Specifications (July 1, 2025)
Submission Templates
NOTE: The submission format for Timely Prenatal Visits and Depressions screenings has changed from the previous year's templates. Below is the one template clinics should use for monthly submissions. For the 2025-26 Measurement Year, there is only one template for both measures. When a member is entered on the template that has a gestational age > 14 weeks, the line of data will turn blue indicating that this member meets the requirement for the depression screening at first visit greater than 14 weeks gestation. The new template includes a separate tab within the worksheet for each month of the year. When submitting the template, please add the new months data in the appropriate tab. This will help keep all your data on one worksheet for the whole year.
