​Comprehensi​​ve Health Interventions for Lifelong Development (CHILD)

Formally Child Health and Disability Prevention (CHDP)​

For facilities that see pediatric patients (under 21 years old), the facility staff are responsible for conducting hands-on-preventive screening and must demonstrate competency and appropriate application of these screenings/services. (DHCS Facility Site Review Standards page 20)

​Please complete each training module. Upon completion of CHILD training module(s), please email FSR@partnershiphp.org to start the process for certificate issuance. Certificates are valid for three years. Please allow two weeks for processing certifications.

​​​​​​Site Review Overview​

Partnership conducts site reviews to ensure that providers have the capacity to maintain member safety standards and practices. A site review includes a review of the physical site, medical records, and a review that evaluates accessibility for Seniors and Persons with Disabilities (SPDs).

Each site is required to pass an initial facility site review prior to joining Partnership's network. Site reviews are conducted at least every three years. A certified site review nurse, using approved review tools from the California Department of Health Care Services (DHCS), conducts the review. Partnership follows standards and guidelines outlined in APL 22-017​.

Site reviews are conducted on a variety of practice types, including PCP, OB/GYN, Non-Accredited, Private Duty Nursing, Street Medicine, Mobile Clinics, Palliative Care, and Substance Use Disorder providers.
