ABCs of Quality Improvement
The ABCs of Quality Improvement (QI) is a one day in person training designed to teach you the basic principles of quality improvement.
During this training, participants will learn concepts behind:
- Introduction to Quality Improvement and the Model for Improvement
- Learn how to create an Aim statement (project goal)
- Learn how to use data to measure quality and drive improvement
- Tips for developing change ideas for improvement
- Testing changes via the Plan-Do-Study-Act cycle
Who Should Attend? Clinicians, practice managers, and quality improvement team who are new or need a refresher on the basic principles of quality improvement.

Capturing Patient Acuity through Coding Webinar - Session 1
This learning session is designed to educate health teams on appropriate coding acuity.
Who Should Attend? Medical directors, physicians, and coding support personnel.
Learning Objectives:
• The participant will be able to differentiate between the different types of codes.
• The participant will be able to distinguish between examples where a patient has or has not been coded utilizing an “acuity" approach.
• The participant will be able to identify complex coding examples and generate patient coding inclusive of a sample patient's complex conditions.

Improving Measure Outcomes Webinar Series
The Improving Measure Outcomes Webinar Series allows Quality Improvement teams to make knowledge actionable, improve quality service, and clinical outcomes around specific measures of care.
These learning sessions will cover Partnership's Primary Care Provider Quality Incentive Program measures. Content will focus on direct application on best practices including eliminating health disparities with examples from clinical quality improvement teams who are doing the work.
Who Should Attend? Clinicians, practice managers, quality improvement teams, and staff who are responsible for participating and leading quality improvement efforts within their organization.
Improving Measure Outcomes: Preventative Care for Children Ages 0 – 30 Months
- Define the clinical background, specifications, and performance threshold definitions of the 2025 Primary Care Provider Quality Improvement Program Specifications: Well-Child Visits for the First 30 Months of Life, Childhood Immunizations Status, Blood Lead Screening, and Topical Fluoride in Children measures.
- Apply measure specification requirements to maximize measure performance adherence in the delivery of well-care visits through age 30 months, screenings, immunizations, blood lead screening/lead screening in children and dental varnish.
- Understand best practices to ensure delivery of blood lead screening/lead screening in children and topical fluoride, including justification for its inclusion in preventative care for 0-30 months.
- Recognize the minimum five components that are necessary for clinical standard practice for the well-child visits for 0-30 months.
- Identify best and promising practices that can be used to address clinical processes, improve interpersonal communication and education, eliminate barriers to access, improve outreach for groups that have been historically, economically, or socially marginalized, and improve technical barriers to improve well-child, immunizations, and related services for children ages 0-30 months.

Improving Measure Outcomes: Preventative Care for 3 - 17 Year Olds
- Define the clinical background, specifications, and performance threshold definitions of the 2025 Primary Care Provider Quality Improvement Program Specifications: Child and Adolescent Well-Care Visits, Immunizations for Adolescents, Topical, Blood Lead Screening, and Topical Fluoride in Children measures.
- Apply measure specification requirements to maximize measure performance adherence in the delivery of child and adolescent well-care visits, screenings, topical fluoride and immunizations.
- Identify best and promising practices that can be used to address clinical work flows, improve interpersonal communication, member and staff education, eliminate barriers to access, improve outreach for under-resourced communities, and technical tips to improve child and adolescent well-care visits, screenings, and immunizations for adolescents, especially for patients from groups that have been historically, economically, or socially marginalized.

Improving Measure Outcomes: Chronic Disease and Colorectal Cancer Screening
- Define the clinical measure background, specifications, and performance threshold definitions for the 2025 Primary Care Provider Quality Improvement Program Specifications: Controlling High Blood Pressure and Colorectal Cancer Screening measures.
- Apply measure specification requirements, including those related to telehealth updates and remote blood pressure monitoring, to maximize adherence and measure performance.
- Evaluate the prevalence and risk factors for high blood pressure and colorectal cancer and associated health inequities prevalent in their diagnosis and treatment.
- Apply documentation requirements to maximize adherence and measure performance in the delivery of colorectal screening.
- Identify best and promising practices including, successful clinical workflows, member and staff education, outreach, and technical tips to improve high blood pressure rates and improve timely screening for colorectal cancer.
- Examine disproportionate prevalence and/or rates of chronic disease and how culture and lifestyle factors (i.e., diet) of groups experiencing risk may contribute to ineffective management.
- Understand Partnership HealthPlan of California's Medical Equipment Distribution Services and the Medi-Cal Rx blood pressure monitor program.

- Identify barriers for timely postpartum care and discuss best practices for implementing effective care practices.
- Identify best and promising practices including access to care, successful clinical workflows, member and staff education, outreach, addressing social context which influence treatment decisions, referrals to local community resources, disproportionate prevalence and/or rates for diabetes complications and technical tips to improve women's sexual and reproductive health.

- Identify best and promising practices that can be used to reduce barriers to accessing recommended cervical and breast cancer screening through addressing clinical work flows, improve interpersonal communication, member and staff education.
- Identify more effective outreach to patients from groups that have been historically economically, and socially marginalized.

- Define the clinical background, specifications, and performance threshold definitions for the 2025 Primary Care Provider Quality Improvement Program Specifications: Comprehensive Diabetes Management - HbA1c Good Control, Retinal Eye Exam, and Blood Pressure Control for Patients with Diabetes measures.
- Apply measure specification requirements to maximize adherence and measure performance for the Comprehensive Diabetes Management – HbA1c Good Control, Retinal Eye Exams and Blood Pressure Control for Patients with Diabetes.
- Evaluate the prevalence and risk factors for diabetes mellitus and related diabetic conditions and associated health inequities prevalent in their diagnosis and treatment.
- Identify best and promising practices including access to care, successful clinical workflows, member and staff education, outreach, addressing social context which influence treatment decisions, referrals to local community resources, disproportionate prevalence and/or rates for diabetes complications and technical tips to improve Diabetes Management HbA1c Good Control rates.
